Calls being sent over PRI with Digium card. Sounds like the PSTN just doesn't forward the name for whatever reason.

Sorry to add another thing. The reason why they do not want everybody to be able to send this information is obvious. You don't want your average joe to be able to spoof their caller id or name to something like "Bank Of America" and social engineer information out of people.
