Forward the calls for the extension to the home or cell phone (or both) in Uxbridge. Then have the boss use a toll free number to dial the Asterisk box, enter a PIN, and connect back out using DISA.

This maintains the PBX for everyone working at home. The outgoing calls via the toll free number are still likely to be less than the $100/month she's paying now.
