Here are a couple of thoughts.

1) If the call to your cell phone is going out a VOIP connection, consider
that you might have a firewall/NAT issue preventing the RTP part of the
conversation from working correctly.  Some searching on voip-info.org will
give you lots of info on troubleshooting this.

2) as for the missing ringback, this is probably indicative of the problem
1.  You don't have a working audio stream to your VOIP provider and so
they're playing the ringback but your caller isn't hearing it.  You can
cheat this by placing an 'r' option in the dial command but I don't think
it's the root of your problem.

Take a look at those things and let us know if that solves it.
