Yes this seems to be what is happening. I kind of timed it and the message
(greeting) does seem to corresponds with the timing (if played twice),
It also seems that if the caller makes it to voice mail then decides not to
leave a message * will record the fast disconnect tone until the mailbox is
full and times out.
So it seems that my problem over all is not been able to detect when a
caller disconnects.
is there a solution to this?

If Asterisk hangs up first it must signal to the Zap channel to go
on-hook( open circuit) Testing this out with a PSTN placed call to a SIP
(ATA) extension proves to be no problem.

If the far end hangs up first then you will hear the silence and the off
hook warning sound. That's no problem either.

the conference the line is siezed forever unless you reboot or use the
