There could be a number of things at play here but I can tell you I've had
the same 1 sided quality complaints and I've attributed it to the difference
in transmit and receive rates on DSL.

I had someone running a small office with a few users on Asterisk and
connecting to a VSP.  People within the office could hear just fine but the
people they called  were complaining that they had a bad connection.  I am
pretty sure that this was caused by congestion in our upstream DSL.

Assuming that you're using ULaw, you could try trading off a bit of voice
quality and going to a compressed codec like GSM.

Sorry I don't have any helpful advice on your echo problem.
