I've had many issues with this over the years. Almost all of our dialing uses IVR at the termination point.

The issue like Phil said it is the the last mile termination carrier where the problem arises, our carriers will try rerouting if they can but the issues still persists, if they all terminate with the same carrier. I'd say it probably isn't because they are gov numbers but where they are located ( New Brunswick ? ). I have a bunch of areacodes in North America that I have had to set up as DTMF inband to alleviate the issue.

Here are the ones that I currently experience problems with:

506 - Saint Johns NB
801 - Salt Lake City UT
210 - San Antonio TX
407 - Orlando FL ( one of the crappiest termination points in North America, not just DTMF issues )

I have also had issues in the past with:
403 - Calgary AB
306 - Saskatoon SK

402 - Omaha NE
305 - Miami FL
