I know that you can do peer to peer encryption of a call, but there's nothing that I know of that authenticates who your calling or who is calling you except the caller ID.. which can obviously be spoofed.
Caller ID spoofing isn't necessarily a VoIP flaw, anyone with a PRI can do it as well as there are websites out there that generate whatever CLID you want, call you and then bridge to whatever destination you want..
