Check the power/cord
Check/replace network cable
If the audio is very loud then it may be overloading the CODEC or Echo Can.
I can't remember if you can control the volume level but if you can turn it 
down (-6db)and try again.
Set the FXO ports to 600 ohms & 2.15 uf
Set-up asterisk to answer after 2 or 3 sec delay (one or two rings)
If you have this set up as a SIP trunk then make sure dtmf signaling is default
to RFC2833 and codec is PCMU (only) If this doesn't work then it may need 
replacing.
