That's exactly how it works. You set the smtp gateway in rc.conf, then when Astlinux boots it generates a /etc/ssmtp/ssmtp.conf file based on that information. The ssmtp package installs a wrapper sendmail that will act just like the "standard" sendmail command and send a message on through to the SMTP gateway specified. As long as Asterisk is setup/configured to use the sendmail wrapper in the right location/path, and configured to send e-mails with the voicemail attached etc, then it should work.
