You can save it as zaptel-tdmoe.patch within packages/zaptel and it will get automatically applied during compilation.

I have just added the zaptel patch found here:
It has been added to trunk in my local copy, I am trying to build with asterisk 1.2.12 and zaptel 1.2.9 right now. It should be committed later today.
