I thought that a compact, easy to use Asterisk would be a good solution for one-man offices like a doctor's office for whom it doesn't make sense to hire a secretary, but still needs to handle incoming calls. I was even thinking of giving out short MD5 hashes when someone makes a appointment through the web or Asterisk, and have the office's ring-bell come with a keypad so that patients must type the unique MD5 hash number they got -> the door opens to let them in, and the patient's profile pops up on the doctor's PC screen :-) Too bad I suck at maths.
