Curious. I can't offer much help, other than either the method used to install the img file is flawed, or the flash is defective. Is it really an "sd" card? I use a simple method to do my installs. I boot the machine with a USB flash with DSL ( Damn Small Linux ) clear the partition table on the CF or DOM flash card ( may not even be necessary ) then use "dd" to install the image from my USB flash to the target. Once that is finished, I reboot without the USB and finish the installation
Only time I have a problem is when the target flash is defective in some way. I mainly use HP thin Clients, 55xx and 57xx series I have obtained used off eBay. Older ones that have DOM smaller than 128 M have to have their DOM replaced 128 M is tight, and requires one to not load many modules normally loaded by default but generally not needed. I use the Transcend DOM that are a direct replacement for the ones used in the HP units, and generally less expensive than a CF adapter and card. The developers have made the install process so simple and painless, compared to the first one I did several years ago ( Painful for me, as I am not all that well versed in Linux ) The install process and the beautiful web interface have constantly improved over the past few years, I am in awe of it all.
