Welcome to Asterisk 13.  They completely re-wrote the CDR engine and now
they log everything, and I mean everything.  Lots of debate on this over on
the Asterisk lists at the time but the bottom line is that there is pretty
much nothing you can do about this.  I have tried using NoCDR() calls in
the dial plan but nothing gets me to a simple one-entry per call CDR.
