In most cases, you only need to set the ESSID, but not the AP. In any case, ESSID should be set last as setting it will trigger the association. It would be useful if you quote the exact output instead of rephrasing the error messages. It's possible that NetworkManager is interfering with the connection. If you can use NetworkManager, then it's probably the best option instead of doing everything manually. If you definitely don't want to use NetworkManager, then I would uninstall it and reboot to be sure.
