It looks like there are some low-level issues in getting frames through
in the AP-to-station direction. This explains why wpa_supplicant debug
log showed some "extra" EAPOL-Key frames. In addition, some of the
station responses are very slow (taking 1000 ms to get out EAPOL-Key msg
2/4 when it should not really take more than couple of milliseconds).

Unfortunately, these are type of issues that can be quite difficult to
debug remotely. I went through all the associations in the file and
could not find any obvious reasons behind the problems apart from the
frames problem in getting the frames sent by the AP received at the
station. Quite a few acknowledgment frames were missing, but I think
some of those were because of the sniffer not seeing them rather than
them not being transmitted.
