How about a couple of the following as random weird reactions that would be of course rare but interesting.
1. you kill a criminal who is actually a criminal, but he's a folk hero among your people so opinion still goes down. This would be more common if you were unpopular.
2. You execute a criminal but the corpse picks up its head and walks away.
3. you inprison someone and they start a prison riot, either killing off prisoners, turning some of your prisoners into enemy bandits, or both.
4. You execute a monster that is seeking refuge with you and this causes an international incident where an independent kingdom comes to hate you and you take a sharp penalty in relationship.
5. someone comes from a mine or other purchaseable location and offers you a good price for it, but it is really a con artist and the money is wasted.
6. the red-skinned man in the coin of fate game is just a guy in costume playing a practical joke on you.
7. a musician that comes to visit you steals some of your gold sometimes if you don't pay him,
8. a musician who visits you is really a thief and when you ask them to play, they swipe some gold.
9. The child that wants the single gold coin swallows it like a small child might and chokes when you give him one coin, thus lowering your public opinion a lot.
10. The performance of a musical abomination frightens away some peasants or soldiers of yours.
11. if you don't judge the beards of the men in the contest, they strangle you with one of them and the game ends.
12. The changelings impersonate you and either change one of your laws at random or do something that decreases your public opinion or on very rare occasions, increases it as they lock you up for a time and impersonate you.
13. Your court employees might come up for some reason and demand you decrease their pay.
14. You have the option to threaten to lower the pay of an officer when they come to demand higher pay. Almost always results in their leaving, but not always.
15. Change the fruit up in the hag encounter when she demands you pay reparations for her missing apple, make it other fruit sometimes.
16. sometimes the public will condemn you for paying reparations saying you are too frivolous or that the claim was not worh paying.
14. When someone comes in to insult you, sometimes they will throw something at you when they are removed from your court.
15. Sometimes when someone comes to insult you, an overzealous guard will kill the person without permission.
16. Sometimes a guard will hate a jester's joke so much that he'll kill him without permission.
