If you hire a shitty quality knight/soldier, or too many sellswords, than there should be a chance that your knights/soldiers will get upset with it and maybe some will defect or you'll lose popularity or something. Maybe this would only happen after a trend of bad decisions though...
If you give significantly more gold than what was asked for by a citizen then you should also get a significantly higher chance of earning popularity.
When staff ask to get a raise, you should be able to barter (with some risk) for half the amount they are asking for, or maybe pay even more and get some popularity, or at least a decreased chance of them asking again any time soon. Though maybe some would take advantage and be more likely to ask you after that...
I think more obvious issues should happen sometimes when killing the hag.
A callback would be nice from someone you helped out, for instance the guy that needs new livestock, where he gets rich and gives you some stuff as thanks a few years later.
I think there should be a bunch of silly random gifts for celebrations and a way to reject or accept them that can effect popularity rather than just gold, since you get gold at the end of the turn anyway.
Rejecting the bandit's offer to forgive outlaws too many times should result in a possible bandit uprising that destroys your prison, loses you gold and some peasants/soldiers, and maybe forms a new pest group.
Throwing game masters or musicians in prison or executing them too much should cause them to boycott you, maybe even in the taverns and guild.
Some powerful mages from the mage guild would be cool visitors, maybe offering some special perks for a high price or threatening you or forcing tribute.
Other kingdoms bigger than you trying to force tribute/vassalization would be nice too, so that they aren't all just at war with you by default.
Arena grand champions should be able to make annoying demands the better their living quarters and wins are.
Champions running forts should be able to come to you with various problems like lazy/routy troops, not enough supplies, requests for assistance against saboteurs and gorilla attacks, wishing to leave their position because of various reasons like boredom/being over worked, or depending on the fort's location, sand flees, scorpions, the extreme cold/heat, fear of demons, bandit archers, ghosts or a bad ancient feeling from nearby ruins ETC.
I think your knightly order should visit you with various stuff they've found, insidiace plots they've uncovered, adventurer groups they've captured trying to attack/steel from you, a corrupt noble fucking up their particular land or abusing their people in some way, slaying a monster ETC.
A champion's internal revolt in the courtroom as a result of low popularity complete with guards dieing and shit would be interesting.
Various different people coming in to lobby to change laws like gambling, land tax, banking ETC, both noble and commoner.
Wedding nobles asking for your blessing, including ones where one side is coerced for power or seems abused, with the public hanging on the outcome.
Courtiers in general asking you for support for various crap and if you choose wrong then problems happen with knight's, champions, popularity do to rumours/blackmail they spread about you, maybe a staff member quitting, a loss of gold or new rebel group with some soldiers of yours, any powerplay like that you can think up basically...
A land refusing to pay tax or declaring independants for various reasons including a popular commoner revolt, a corrupt noble ETC, which if put down badly will cause popularity/peasant problems.
That's all I can think of for now.
