You have a ground loop and no amount of power line filtering will
eliminate it.

The problem is that the cable-tv coax is grounded at one loction and
the electric outlet ground is at another location.  You don't need
something to isolate the sub input.  You need something to isolate the
ground from either the sub amp (third wire on power line) or the coax
line.  The sub amp ground is there for safety and should not be lifted.
The cable tv coax is another story.

You need a 75 ohm isolation transformer that will break the DC
connection between the coax ground and  your audio/video system. 
Search for 75 Ohm isolation transformer and you'll find what you need. 
Of course, if your have a satellite receiver on that coax you need to be
able to send DC to the LNA to tune the receiver.  I think there are
isolating transformers that solve that problem also.
