I use an irock Beamit in the car.  I see it's been discontinued, the
equivalent would be the 410:

The only problem I have with it is that it picks up ignition coil noise
- you hear a whine which changes with engine RPM.  To get over this, I
put my playback device at 100% volume and use MP3Gain to set my MP3s at
95 dB (!)  This maximizes signal-to-noise so I don't have to turn up my
car radio that loud.

irock used to sell a filter which would eliminate this noise but I have
enough bulky gear to carry around.

Apart from the ignition coil noise, the resulting sound is surprisingly
good.  Some clipping caused by my overly high gains but still quite
good.  Better than a local FM station.  The signal has never gotten
knocked out by a stronger signal and I don't get static or pops.  In my
vehicle, the aerial is located quite close to the transmitter though -
if I had a newer vehicle with the aerial at the back this might change.

I tried it out in a friend's vehicle on battery power.  This didn't
work very well - I had to hold it up in the air or else the signal
would fade.

My playback device (Dell Axim X30) generates a little EMI on its own, I
can hear it if the engine isn't running and the processor does something
intensive like plot a GPS route.

I've seen a few car radio units that have stereo submini AUX IN
connectors, those would be fabulous!  Just a line direct from the
player to the radio.
