MS Access works wonders.  I developed an inhouse dB that tracks and reports and works very well (the regulatory bodies like it as well!)

When an audit report is issued I perform two additional steps:  Add estimated followup time into my annual audit plan as a comment.  In Microsoft Outlook, schedule specific followup tasks for some time in the future.  This way, my audit plan is earmarks the manhours needed for followup and Outlook reminders "pop up" when it is near the time for followup.
