Yes that nicely illustrates the SYSTEMIC problem when people are indoctrinated to believe that there is a chain of responsibility with competent people above them, instead of being told from the beginning that when they are pilot in command the buck stops with you.

The other systemic problem is the continued existence of instructors like that and despite the formal existence of the instructor panel/RTO/CTO/GFA Board the lack of actual supervision and responsibility of those people.
