Why don't you use the wording from the GFA form?
>>> [EMAIL PROTECTED] 19/10/01 10:30:05 >>> I'm interested in getting some examples of the types of "indemnity forms" that are in use. The kind of thing that says "I know it's risky and I promise not to sue". I don't want to open a debate on the value or otherwise of these forms, just to get some examples. Would appreciate it if anyone has any they can pass on, could you either send me as an email attachment direct to "[EMAIL PROTECTED]", or else fax to the number below.
