The only book I have in one ODT file is for the printed edition
of the Writer Guide, which is put together from a different set
of chapter files, formatted for a smaller page size and with some
other differences. If it will help you, it is here:

My difficulties are:
Normally in a chapter document you have "OOoHeading 1" to 3 for
chapter layers. But now there are two more layers above, Chapter
(of the Draw guide) and Title (of that Chapter). Okay, I already
thought of just creating those and pushing OOoHeading 1 to 3 two
layers down. Then I will have to correct this in the toc, too.
I use only one layer for Chapter and Title, because I have too
much trouble with the layout of the toc if I use 2 layers. Then I
pushed OOoHeading 1 to three one layer down. I have not have any
problems with doing it this way.
