[authors] Updated: Chapter template, Template How-to, and Writing OOo Guides

I have updated three files in the Contributors section of the 
website: Chapter template, Template How-to, and Writing OOo 
Guides. In addition to accepting changes made by TJ Frazier, I 
made some other minor changes and additions.

Among other things, I removed the colon at the end of the chapter 
title. This is because, when the chapters are put together to 
form a full book, the chapter title (from the OOoHeading0) goes 
into the page footer but the chapter subtitle is not included. 
Having a chapter title ended by a colon in the footer looks 
silly, so I removed the colon. It isn't needed on the title pages 
of chapters with subtitles anyway.

Apologies to TJ for taking so long to get around to dealing with 
his changes, which he had done at the beginning of January. I was 
so focused on finishing the third edition of the Writer Guide, 
that I forgot about everything else.
