2 March 2008-- (1) Copyedited.
(2) As with Chapter 3, an index was appended to the back so that the
indexing of the chapter could be checked. There were two upper/lowercase
errors.
(3) Numerous character style changes made.
(4) Some minor technical errors.
This chapter should be ready to publish after the edit changes are
eventually accepted. In the interim, they are left for any review.
