We don't really have a policy about including third-party extensions, but this is my rule of thumb: I only include those extensions that I personally use and find to be good value. Usually those also happen to be produced by Sun or by members of the community (Template Changer, for example) and are open source.

If one of those extensions stops working when OOo is upgraded, then I would either remove it from the manual or (if it's particularly important, like PDF Import) note that it doesn't work for OOo version xxx (or on xxx platform, or whatever).

In the specific case of DataForm, I would remove it from the manual. It's too minor an extension to include if it is no longer compatible with the current version of OOo.

If I encounter this again, should I ask users on this list to test, or post
the request in the OOo Forum?

I usually start by asking this list and/or the Docs list, because usually someone here knows the answer to my question. If that fails, I go to the forum. Depends on the question, though. Use your own judgment or preference.
