There are two churches in Vila Franca do Campo (most researchers just type
Vila Franca).  One is Sao Miguel Arcanjo or the Matriz (the mother church).
The other is Sao Pedro.  Your Bernadina is from S.Miguel Arcanjo.  I found
it on the immigration out of the Azores here (use Internet Explorer; Firefox
doesn't work):

To obtain a copy of her baptismal, you'll need to write to the Biblioteca on
Sao Miguel island.  The address and a fill-in the blank letter can be found

BUT, you will notice that you have a birth of 1896 and she's saying she's 18
in 1912.  So there's an age descrepancy there.  I'd do a little bit more
sleuthing before writing to the archives and asking.  The archive/biblioteca
in Ponta Delgada on Sao Miguel will look up what you ask, but not do a
search.  So see how old she is at her marriage, on the censuses, etc.
