I'd title it "Pedido de ajuda para contactar a Editora Guarda-Mor" (Help to contact the Guarda-mor publishers). That would be the 'Assunto' (subject) of the message, and then just write the message in English. Everyone will understand. Post it to the 'Bibliografia' category.
