Hi Drew, Even though I only visited those islands once, I can attest to the fact that Horta is the only "real" urban center in those two islands. There's even a modern supermarket near the new hospital in Horta that I'm pretty sure stocks up on shoes and just about everything else.
I'm pretty sure that most 'active' people in Pico do not spend more than a month without taking the boat between Madalena and Horta.
