I think your translation ("being attacked by sudden death") is right on the
mark. My transcription/translation is below.
Faleceu sem receber os sacramentos de Santa Madre Igreja por ser atacada
morte subita.
Died without receiving the sacraments of the Holy Mother Church for being
attacked by sudden death
