My two cents worth.
In my experience they ask for NOME and SOBRE NOME.
And also in my experience Apelido and Alcunha can be interchangeable.

Well perhaps, but since I got into doing genealogy 20+ years ago, alcunha was 
reserved for nickname.  And after that poor guy at that table flipped the words 
alcunha & apelido, I just thought that alcunha confirmed what I learned.  
Perhaps in some families they use one more word more than the other.  Seems 
more families called the nicknames alcunhas though in my research.  Someone 
would have to do a study to figure out the true percentages though.

What do Portuguese employment applications ask for though?  Maybe one of our 
listmembers there know.
