Hi Sue, I see "...os Divinos Sacrament.os Ig.r ser quase repentina sua morte..." and with google translate's help, I interpret that as "the Divine Sacraments of the Church [because] her death was nearly sudden". I can't transliterate the second section that you asked about, but my impression is it says something along the lines of: "she was accompanied by [someone/representative] from the town, from the Reverendo Collegio of the Matriz"--that is,someone from one of the lay brotherhoods whose members participated in religious ceremonies went with her body as it was carried to her grave. Perhaps someone with a better eye can confirm or correct this.
