The way the name is written is a little strange to me, it looks like Seb.am, so I'm not sure it is him but think so. The date ties with what is on the UMinho site.
Date of death is 09 Oct 1719, and his age is 65 years more or less. Beyond that, I cannot make out much of what it is saying.
