This priest's handwriting is very difficult to read so I can't be of much help, but I do believe the name is Sebastiao de Mendoca, married man, although I can see where it named his wife. I think at the end of the 3rd line, extending into the 4th, it says he received the sacraments probabaly of the inferm, but I can't make it out clearly. It also says in the 4th line that he left a will but I can't make out what all the names are and what they mean. I believe what follows below may be listing what he left. I see 120 somethings for his soul, thousand reis.... He was interred in the church of the Apostolo de São Mateus, where he was a parishioner. Very difficult to read. So sorry I can't be of more help but I'm sure someone else can add a lot more. Good luck!
