Hard to say... might be hardware-related. In the simplest case, it's the SCSI cabling or termination, so you should check that. Also, see if there is a firmware update for your tape drive. Then, try another SCSI HBA or driver - that might also cause problems. (And I hope you don't have the tape drive attached to a RAID HBA or together with your hard disks on one channel...)
