The rev 01 cards work fine for most people. It is the rev02 ones that are a problem. As I have stated many times, bcm43xx uses V3 firmware. Getting the V4 driver you listed above will do nothing for you. I have no idea what is wrong with that kernel. What information comes from 'dmesg | grep bcm'?
