Apparently enough died for an RMA to be accepted. I got it replaced, now the new card is recognized (although I still can't associate with the new b43 driver, only with bcm43xx - it gives "Authentication timed out", and Packetspammer doesn't transmit anything in monitor mode, while kismet works fine, indicating no Tx, but working Rx). I am now using a kernel without SMP and PAE enabled, so that can't be the bug.
