Foobar2000 works quite well for this. It can use the information based on your cue sheet to create individual files in whatever format you want (flac, mp3, ape, etc.) with whatever naming convention/directory structure you want. Basically, foobar2000 will read the cue sheet and present a list of tracks. You can select all of the files, right click, and do a convert. Convert to FLAC if you want to keep them in flac. Under options, you tell it what naming convention to use for the files based on the tags.
