Exploring this a little more, it does look like a new & changed scan finds the new artwork. The problem appears to be in a couple different places in the web interface. When I browse albums I get the placeholder and then also on the albuminfo page. The trackinfo page is fine. It's not a browser caching issue, as I cleared the browser's cache and I also tried in a browser that I seldom use.
