Are you thinking of the issue with the windows version of faad.exe, that
had a problem with some on-line aac streams with the HLS bbciplayer
plugin?

No, mp4 files streamed from a remote source. I'm trying to find an authoritative answer online. I only found some internal communication which was clear about this when implementing some service years ago:

"MP4 - they should not be streaming that. MP4 is not a very good choice. No Transcoding and no seeking"

Unfortunately they forgot to mention _why_ this is. But I think it had something to do with where some of the file's metadata is stored. There's a problem when a mp4 file is not available for random access (as with local files).
