A template can never know by itself whether it's used for a first or later 
citation, that should be obvious. The only way to even theoretically have this 
information, is when you use some kind of program to parse the document and 
insert citations in a context sensitive way. Such as bibtex. In fact, this is 
not only theoretically true, bibtex does have some of this capability to do 
this through AppleScript. So this is not a limitation of templates, it's a 
limitation on the method you use to write your documents (i.e. wysiwyg). There 
was a set of AppleScripts or an app that could do this for a specific editor, 
perhaps BibFuse or CiteInPages.
