Perhaps Conan Albrecht can modify BibDeskToWord to allow for separate
templates for first and later citations. Until then, perhaps you can use the
following trick: insert a \cite citation for the first citation, and \citet
for a later citation, because BibDeskToWord does allow separate templates
for those two different commands. You can insert different citation styles
using the citation drawer.
