I think a screenshot would help (cmd-shift-4, hover over main window, hit 
spacebar).  Also, check Applications/Utilities/Console.app to see if any 
exceptions were logged.

Can't you enlarge it?  Switch the lower pane of the main window to show icons, 
and zoom in (there's a slider that appears as you hover over the top of the 
icon view).

Again, double-click that wrong icon to see if the wrong document opens.  That 
might rule out some of the weirder possibilities.
