That is because the way iPadOS works. All the app are containerized
including Dropbox. The only app I know it can access other apps containers
is the Files application (which is part of the OS).

If that is the case, this means that Ref42 cannot access the container of
Dropbox in the iPad. But it needs to download a cached local copy which it
open with the system PDF viewer.

I think the only possibility to do what you ask would be to have Ref42 have
the option to upload the annotated file automatically on Dropbox and
substitute the original file. That depends if the Dropbox API allows that.
Simply upload the file with the same name will loose sync on BibDeak,
because the file is linked with the descriptor, not the filename.

Not sure how easy it is but we could ask the developer if he can take a
look at it.
