Hi,
When (using Outlook XP) I attempt to create a folder at the root

With a Maildir++ depot, you must create all mailboxes under INBOX.
Try creating INBOX/foo.".
This makes sense, as far as I can tell, given the way that Maildir++
folders are structured.
What I'm wondering is if I can create Maildir/.Inbox, and have INBOX
mapped to that,
No, check out http://www.inter7.com/courierimap/README.maildirquota.html
for the Maildir++ specification, which says that INBOX is always mapped to
the root Maildir directory.
and then I can create other folders in the root, such
as Maildir/.bincimap-list for example.
Using a Maildir++ depot, this maps to INBOX/bincimap-list in the IMAP
client. Would it be possible for you to switch to an IMAPdir depot? This
is what I use and it works well with mutt.
This is currently how I have my ~/Mail folder organised (as a set of
recursive Maildirs), and it works well in mutt. I'd like to do
something similar for Outlook.
Unfortunately, you can not map IMAP folder hierarchies into Unix folder
directory hierarhies using either Maildir++ nor IMAPdir depots due to
semantic differences between IMAP folder hierarchies (as specfied in RFC
2060) and the Unix directory structure. Please check the bincimap
archives for further discussion on this topic. My suggestion is to
experiment with an IMAPdir depot to see how close it is to meeting your
objectives.
Thanks,
Roger.
