Howard,
Many thanks for you help with this, it was because the other
contacts didn't have email addresses set for them!
To answer the rest of your questions in case anyone else makes the
same mistake:
I can get to the rest of my contacts in the first pane by bringing
up the address book then pressing right-arrow
The Address Book option is checked and shows the same number as my
address book
The recent email addresses option is not ticked
Gmail Notifier is set as the default app in Mail.app's preferences
In Quicksilver's handler sections the email handler is set to Gmail
Notifier (I have this or Mail to choose from)

Can you see the 100 contacts in the first pane?
If you bring up Address Book in the first pane and type right-arrow do
you see all the contacts?
Do the 89 other contacts that don't appear in the 3rd pane have email
addresses set?
In the QS Catalog under Modules, next to Apple Address Book is the box
checked and is there number to the left of the checkbox? Below that is
an entry for Recent Email Addresses, is this checked and by any chance
is the number 11 to the left of the checkbox? Perhaps this is the
source of the contacts.
When you say "Gmail notifier is set as my default email app" what do
you mean? In Quicksilver or in Mail.app Preferences (where OS X
mysteriously puts the UI to set the default email app). "Gmail
notifier" is unrelated to using Gmail in quicksilver. In the QS prefs
under Handlers for Email I see choices for "Mail" or "Gmail Module".
