QUicksilver's volume controls are just plain Applescripts. This means that QS isn't doing anything directly to the volume settings, but instead is just asking the operating system to change the settings. Now, why the operating system seems to be confused about your volume settings I don't know. I suspect a simple reboot would be a quick way to fix the problem.
