Just for the record I am having the same behaviour on Leopard. What's
weird is that I only have it on doubled entries. And the duplicate URL
entry display certain encoding errors like when you try to view a
UTF-8 encoded string as Latin-1 or MacRoman. This falsely encoded
string is also the one displaying the non transparent default icon on
top. Not every URL is doubled though. I have been unable to determine
which criteria causes the URL to get duplicated and attached a false
icon.
