I suspect you're seeing two different (sets of) actions Email To... and Email 
Item... which are similar but different. They're used as followed:
document Email To... Howard
Howard Email Item... document

check the Email section for details.  

Or it could be a bug that sometimes the icons are wrong. This sometimes happens 
with the mail actions or in the first pane with URLs or in a few other places.

I usually describe QS as easy to use but not intuitively obvious. There's a lot 
there and it's hard to figure out. This compelled me to write the manual and 
made structuring it difficult. I included the "How To Read this Manual" section 
at the front to help explain what I did. But given such a question you should 
be able to search the manual for the action name or skim the table of contents 
for the topic (email would be a good guess in this case) and jump there to find 
everything on that topic.  Unfortunately the Actions preferences section in the 
manual is still TBD.
