I've noticed this myself for months and was about to say “You're not alone but 
I have no idea how to fix it”, but I just started theorizing and looking at my 
catalog and I think I've figured it out.

Do you have “Recent Objects” selected in your catalog? (It's under the 
Quicksilver section.) Turning that off (and rescanning the entire catalog) 
seems to have eliminated the duplicates for me.
